Form Images Setup

Print a Patient Invoice or Statement with a specified image.

Form Images Setup allows the ability to change what image is included with Patient Invoices or Statements when using the "Print with Form Image" option in the Print menus.

The following images are used as defaults, but can be changed at anytime.
