Patient Care Hardcopy Signature Documents

Hard Copy Signature is a one-page document which allows you to select a signature type (which includes the signature agreement text and signature).

Hard Copy (with Redaction) includes all information a normal Hard Copy with however will show “Redacted” in place of the following information:  Patient Name, Date of Birth, Weight, Social Security Number, Race, Subscriber, Address, Phone, Physician, DL State, DL number and Doctor.  Also, all Insurance Information, Residence Information and patient or Representative names or signatures will be “Redacted” as well.

Hard Copy Supplemental includes all supplemental fields listed in the PCR Supplemental Tab.

Patient Care Number Change Audit Log is a 1-page document which includes User, Field Name, From Value, To Value, and Change Date.

Patient Care Audit Log is a document which includes Audit Date, User, and Audit Type.
