This section documents where the patient was taken, the type of transport, and the disposition of the unit, patient, and crew.
Taken To Location
Taken to Location Name - Required when using AIM Billing (eDisposition.01)
- The name of the facility, entity, or location where the patient was delivered or transferred
- Free text field
Location Code - Optional (eDisposition.02)
- The type code associated with the destination location
- Used to identify the specific facility code or identifier
Address - Optional (eDisposition.03)
- The street address of the destination
Address 2 - Optional
- Additional address information (suite, apartment, etc.)
Type of Destination - Required (eDisposition.21)
- The type of destination the patient was delivered or transferred to
- Options:
- Alternate Care Site
- Assisted Living Facility

- Drug and/or Alcohol Rehabilitation Facility

When Type of Destination is equal to "Hospital-Non-Emergency Department Bed", the following field, Destination Hospital Department, will become highlighted - users must specify which department the patient was transferred to using this field when that information is available. - Identifies whether patient contact was made by this EMS unit/crew
- Options: Cancelled on Scene, Cancelled Prior to Arrival at Scene, No Patient Contact, No Patient Found, Non-Patient Incident (Not Otherwise Listed), Patient Contact Made
Patient Evaluation/Care - Required (eDisposition.28)
- The type of evaluation or care provided to the patient
- Options: Patient Evaluated and Care Provided, Patient Evaluated and Refused Care, Patient Evaluated (No Care Required), Patient Refused Evaluation/Care, Patient Support Services Provided
Crew Disposition - Required (eDisposition.29)
- Documents this crew's disposition regarding patient care
- Options: Assumed Primary Care from Another EMS Crew, Back in Service (Care/Support Services Refused), Back in Service (No Care/Support Services Required), Incident Support Services Provided (Including Standby), Initiated and Continued Primary Care, Initiated Primary Care and Transferred to Another EMS Crew, Provided Care Supporting Primary EMS Crew
Transport Disposition - Required (eDisposition.30)
- Documents whether and how the patient was transported
- Options: No Transport, Non-Patient Transport (Not Otherwise Listed), Patient Refused Transport, Transport by Another EMS Unit/Agency, Transport by Another EMS Unit/Agency (with a Member of This Crew), Transport by This EMS Unit (This Crew Only), Transport by This EMS Unit (with a Member of Another Crew)
Reason for Refusal/Release - Optional (eDisposition.31)
- Free text field documenting the reason for patient refusal or release
Disposition Information
Level of Care Provided per Protocol - Recommended (eDisposition.32)
- The level of care provided to the patient based on protocols
- Options: ALS - AEMT/Intermediate, ALS - Paramedic, BLS - All Levels, Critical Care, EMS and Other Health-Care Staff, Integrated Health Care, No Care Provided
Transport Mode - Required (eDisposition.17)
- Indicates whether the transport was emergent or non-emergent
- Options: Emergent (Immediate Response), Emergent Downgraded to Non-Emergent, Non-Emergent, Non-Emergent Upgraded to Emergent
Transport Descriptors - Optional (eDisposition.18)
- Additional descriptors for the transport mode (e.g., lights and sirens usage)
Transport Method - Required (eDisposition.16)
- The method of transport used by this EMS unit
- Options: Air Medical-Fixed Wing, Air Medical-Rotor Craft, Ground-Ambulance, Ground-ATV or Rescue Vehicle, Ground-Bariatric, Ground-Mass Casualty Bus/Vehicle, Ground-Other Not Listed, Ground-Wheelchair Van, Water-Boat
Number of Patients Transported - Mandatory (eDisposition.11)
- The total number of patients transported in this EMS unit during this event
Acuity Upon EMS Release of Patient - Recommended (eDisposition.19)
- The patient's acuity level when care was transferred or released by EMS
- Options: Critical (Red), Dead with Resuscitation Efforts (Black), Dead without Resuscitation Efforts (Black), Emergent (Yellow), Lower Acuity (Green), Non-Acute/Routine
How Patient Moved to Unit - Optional (eDisposition.13)
- The method by which the patient was moved to the ambulance
- Options: Assisted/Walk, Backboard, Chair, Carried, Other, Stairchair, Stretcher, Wheelchair
How Patient Was Moved From Ambulance - Optional (eDisposition.15)
- The method by which the patient was moved from the ambulance to the destination
- Options: Assisted/Walk, Backboard, Chair, Carried, Other, Stairchair, Stretcher, Wheelchair
Position of Patient During Transport - Optional (eDisposition.14)
- The patient's position during transport
- Options: Fowler's, Lying on Side-Left, Lying on Side-Right, Prone, Sitting, Supine, Trendelenburg
Reason For Choosing Destination - Recommended (eDisposition.20)
- The reason why the specific destination was chosen
- Options include: Closest Appropriate Facility, Crew's Decision, Family Choice, Law Enforcement Choice, Patient Choice, Patient's Physician Choice, Payer or Insurance Company Choice, Protocol, Specialty Resource Center, Standing Orders, Transfer Orders/Interfacility
Hospital Capability - Optional (eDisposition.23)
- Special capabilities of the destination hospital
- Options include: Burn Center, Cardiac Catheterization Lab, Cardiac Surgery, Hyperbaric Oxygen, Pediatric Trauma Center, STEMI, Stroke Center, Trauma Center (Level I-IV)
Instructions Provided - Recommended (eDisposition.26)
- Instructions or information provided to the patient or receiving facility

- Documentation of hospital team pre-arrival alerts or activations
